1/       Vouchers are for an at home whitening kit provided under prescription from one of our dentists

2/       Tooth whitening home kit will only be provided for patients that are deemed dentally suitable following a full consultation with the dentist and upon completion of a full assessment and consent form. Patients do not need to be registered with our practice.

3/       Only suitable for patients aged over 18

4/       Results are variable and are not guaranteed

5/       Whitening does not change the shade of veneers, crowns or fillings and these may need replacement following whitening.

6/       Should aesthetic treatment be required to match the new shade of the teeth after whitening [replacement of crowns, veneers, fillings, bridges, etc.] charges will apply

7/       If you are not suitable we are happy to transfer the value of the voucher to another person

8/       No cash equivalents will be given

9/       Treatment will be as follows; initial consultation and if suitable impressions will be taken of your teeth, a second visit (usually a week later) to demonstrate procedure and fit whitening trays

10/     You may require to visit the hygienist prior to beginning treatment – standard NHS charges will apply
